The Executive Board will meet to review the paper work, and statements of need that are received.  They will decide if all, or part, of the request will be taken care of by our Gulf Coast M.S. Society.   It will speed up our work if all paper work required of MSers is completed prior to submitting them to the Board.  Here are some very important steps that must be completed for the Board to act on a Mser’s needs:

 

1.  The Mississippi Gulf Coast Multiple Sclerosis Society will serve MSers

Primarily in Hancock, Harrison, and Jackson counties.

 

2.  MSers requesting assistance for the first time will need a letter from their

Neurologist, confirming a diagnosis of Multiple Sclerosis.  This letter must

be turned in to the Board BEFORE any request can be considered.

 

3.   We will send you a Request for Assistance form that will need to be signed.

This form gives a description of need, and is a must.  No request will be

considered without this form being signed by the M.S.er.

 

4.   A copy of the prescription (if the need is help with medication) and the

name of the pharmacy you are using.  A copy of all invoices for requested

assistance must be included with the above form.

 

5.   If the need is for equipment (walker, bath chair, wheel chair, etc.) a letter or

note on a prescription, signed by your doctor stating medical need is due to

M.S. will be required.  An evaluation will be needed for larger equipment.

If possible, estimates of cost from more than one supply store.

 

6.   If you are able to meet part of the cost, such as with insurance and/or partial

payment, send copies of receipts/canceled checks and/or insurance papers

(Explanation of Benefits form) to show those payments.

7.   All information given to an individual Board member will be presented to

the full Executive Board.  The Board observes strictest confidentiality.

 

If the Board needs any other information concerning your request, it will contact you.

A letter explaining need, or circumstances will give the Board a better understanding of your need for assistance.  So include a letter, written in your own words, to give us better insight as to your situation at the time of need.  We will be able to make our decision quicker if all of your paper work is in and complete.

If the Board can assist you with your request, a check will be issued in the name of the pharmacy, medical supply store or business you are purchasing your equipment from.  Therefore, it is important to supply us with this information.
